The actual name is "The Everyone Everywhere List".  It's a booklet, not a
book, and doesn't have an ISBN.  It is put out by Magic & Tactics Unlimited,
stock number MTU 1010, but there is not an address for them in it.  I
believe it is still available through gaming stores because my local one
carries it.  The MSRP is $7.50 but they sell it for $5.
