In a similar spirit, I'd like to see whether or not swaps were
available or taken in the game record.

I think (1) is already discriminated from other losses, as Forfeit vs.
Resign.

Implementing (3) would require
a). game wide code to do the concept (in both logging and reading the
   game record), and
b). a per-game hook in those few games where it arises.

(4) is not only a whole new concept, it also doesn't guarantee the
info you want if it's voluntary (and I doubt forcing people to say
why they resign would go over big:-)
