Could be all the conditions you described.  Mainly BYB have little knowledge, 
or respect for the history, standard, or future of the breed.  More casual 
breeders who just "want to breed their dog".  They would buy the cheapest and 
breed to the cheapest stud or brood bitch.  Little or no knowledge or 
interest in the unique qualities of the breed they are breeding.  Breeding is 
not a casual thing.  It is hard and the more knowledge one has the harder it 
becomes.  Most of us know the BYB when we enounter them.  A BYB is not 
necessaruly a puppy mill.  These a purely commercial when price and 
produciton are the only important parameters.
