I'd recommend using Sam's Teach Yourself HTML 4 in 24 Hours.  I have the
predecessor book, and it's an excellent reference.

Macromedia Dreamweaver.  It's as close as you can get to hand-coding without
actually hand-coding, but you should know raw HTML first.  It's available
for both Windows and Mac.  You can download an evaluation version from
Macromedia's site.
