How about every full moon his arm gets posessed, and any animals or even people it touches has a chance to pick up the ailing chronic hinderance? also any food becomes tainted with some sort of plague. in fact dont let the arm get posessed it just becomes a plague arm. when he eventually figures out what is happening he has a choice, cut it off or find a cure.
