I'll respond to the HoE side, as it deals with the future.

IMO, and out of game - The reason that Biren sent the Aces out was because
the character was going to be the servant of Pestilence.  The whole disease
creator from the Black Death to the Texas Tummy Twister thing I think
points to that.  For some reason, PEG decided that they needed to stick
with Raven/Stone/Grimme/Hellstrome (maybe to match the aces in the poker
deck) as the big four lieutenants of the Reckoners and they let Biren slip
by the wayside.  I don't know about you, but I thought Hellstrome was being
groomed for War, myself. (The whole Hellstrome = Pestilence because of
Biological Warfare felt pretty weak)
It may be the same kind of retro-fitting that happened when they capped
Grit at 5, but they had already published a couple of things that happened
when 10 grit was reached.

Now - within the game, this may be explained as either:
	A) Biren WAS the primary bat-boy for Pestilence, but was destroyed by a
heroic posse who was helping the leader of the Lady Luck Society destroy
his old nemesis.  (expect the adventure soon).  Pestilence, now missing his
primary servant, now takes Hellstrome away from War and makes him his own,
whispering a different kinds scientific discoveries in his ears  - leading
into the biological weapons first used in WWI.
	B) Biren was working for Pestilence, but he never really understood who
his master was.  All he knew was that the beings that granted him power
wanted him to create these diseases and occasionally told him to "hunt down
X".
