I'd allow the character to get it fixed, but I wouldn't let them start
with a replacement.  Meaning, if you start with a missing arm, then
you've got a Hell of a lot of work ahead of you before you save up enough
to have a replacement put on in Junkyard.
Some have said that those missing limbs shouldn't be able to be replaced
at all, saying that extensive nerve damage or messed-up healing prevent
it.  I think that may be going a bit too far, since it's already really
hard to get a new part in-game, and if that many people had lost limbs in
ways that made them irreplacable, then the cyber limbs wouldn't be very
good prosthetics in the first place.
