I would treat it like an explosion and follow the guidelines for dodging
and massive damage in Waste Warriors.

I have always pictured it as a ball that expands from a single point to
fill the target area. I think the text supports that (something about
"anywhere within range" and no mention of intervening cover).

If the syker could see inside (through a window) then should probably be
able to activate the power in there (yeah, it's nasty, but at least it's
contained). If they couldn't, or activated the power outside I'd say that
the people inside would get normal cover/armor.
