From what I recall of the latest massive damage rules (I don't have a rules reference handy right now), you cause the full damage value to 1d6 hit locations in the first burst radius, 1d6-1 in the second, etc. until you get to 1d6-6. The hit locations are determined randomly, so you can easily get a limb blown off if you take damage to the same limb three times in a row, as what happened to my Posse's Doomsayer.
It's certainly deadly, but then so are explosions. On the PEG HoE website they've reduced the damage of some things that cause Massive Damage:

You'll notice the Nuke miracle now starts at a base 3d12 damage, for example.
