Our vet told us to have Ceasar's liver and kidney's checked every six months. Caesar's hips are so bad at this point, and the pain has gotten so bad without the medicine, that it is worth the possible side effects.  At his age, and given the pain he was in, pain prevention has become our primary goal. 
  If he was younger, and in less pain, I would be much more concerned with the long term effects of an NSAID.
