Joseph's comments were very good. I would say that you and the player
should architect the personalities together, then you, the marshal, should
decide when a personality manifests itself. You can have a set of
conditions that trigger certain personalities, and you can make certain
that these are, in fact, hindering to the character and the player.

That does the best job of simulating the real disorder in the game.

And if that doesn't seem to be worth the work (it may not be), just roll a
d12, like you said. :-)
