Sounds like a food intolerance or actual allergy that has become worse with age.  Find a vet who understands such things.  Both the hot spots and GI symptoms are consistent for same.  The sooner the better.  At 12 not sure how much their system can stand if it is being "attacked" by an build up of incompatable diet.
