[WW] FYI Player Survey

at the start of my campaign (my WW game runs over the winter months, 
concluding around may, then refires in october-married with children syndrom 
and all), i give each player a "survey" to quickly fill out to help me get 
some idea in how i should run my game. it seems to work well and sometimes 
the results can be quite interesting.  the survey includes the following:

In order to better set up the campaign in a format/style that all players 
will enjoy, please HONESTLY fill out the following questions (in an American 
dialect please) and return them by the end of tonight’s game.
