Occasional lapses during the first year are to be expected.  When they
occur, just go back and reinforce your original housebreaking program.  As
he matures, the lapses will get less and less.

Lapses can also occur as a result of 'upsets' or stresses such as being
unwell, changes in family dynamics, Christmas, quarrels, changes is attitude
towards the dog, punishing the dog 'unpredicatably' (in the eyes of the
dog), body changes as a pup reaches sexual maturity etc, etc.
